FEES AND PAYMENT POLICY:
In an effort to keep dental costs down while maintaining a high level of professional care we have established the following payment plans for the use of our patients.
Fees may be paid as follows:

1. Prepayment , or payment at time of treatment with a 5% discount.
2. Payment of balance in full upon receipt of statement.
3. Use of your CareCredit, DISCOVER, MASTERCARD, or VISA credit cards.
Extended payments are available in special circumstances. Accounts outstanding more than 60 days from treatment date will bear interest at 1½% per month or 18% annually.
All major treatment and treatment involving a laboratory procedure will require an appropriate down payment.
Our senior citizens (62 and over) will receive 10% discount for payment at time of treatment.

If you have insurance we will help you to determine the coverage you have available. We ask that you assign your insurance benefit to us. The balance will be arranged for you to pay as listed above.
Professional care is provided to you, our patient, and not to an insurance company. Thus, the insurance company is responsible to the patient and the patient is responsible to the doctor. We will help in every way we can in filing your claim and handling insurance questions form our office on your behalf.
